There's a difference between 'worn down' (used) and 'failed' (broken). A worn tool just needs to be replaced sooner. A failed tool can damage the hammer itself.
Check for cracks that run perpendicular to the length of the tool. Those are fatigue cracks from hitting things at the wrong angle. If you see one, reject the hammer.
